(i) to evaluate your health and provide educational feedback

(ii) to evaluate the effectiveness of behavior change and health promotion programs

(iii) to suggest recommendations for future health programming within Make Me Healthy, or another organization, business, educational, or government body

(iv) to compare the health behaviors and risks of groups with each other and national data

(v) to compare the health behaviors and risks of different segments of the population

(vi) to apply for funding for programs that will help promote health among individuals and families that participate in those programs

(vii) to investigate the relationship among lifestyle behaviors, knowledge, attitudes, and mental and physical health and well-being

When used for purposes (ii) through (vii), your data will be combined with the data of other respondents and subjected to aggregate data analysis and reporting. No identifying information will be included in such reports. Please read each of the above statements carefully before choosing to enter any personal data into our surveys.
